We've seen games like this where big second half leads ultimately fail, and it's usually due to muffed punts and crazy Brady lame duck throws to no one.
So far we've seen both--and the whole Cyrus Jones thing is Belichick's mistake. The dude has touched the ball a total of 16 times on returns and fumbled on 5 of them. That is inexcusable for both player and coach.
