I've seen it more than twice in several ways - our coaches just do stupid stuff playing the Pats.
This (rehashed): http://deadspin.com/all-the-falcons-had-to-do-was-not-lose-23-yards-1792028183
Falcons in field goal range with 8:31. Play the percentages, run the ball, then kick the field goal. Game over. Instead, they pass, get pushed back for a 51-yard attempt, miss.
Stories like this over and over.
The coaching staff spends anywhere from 80-120 hour weeks during the season, so it's embarrassing when this kind of FAIL happens.
May I suggest while BB may be a football Einstein, most of his success is simply detail in doing common sense things? And conversely, the failure of his opponents of due diligence? Knowing your players, knowing your opponents on game films, knowing what you/they do on any given situations and the percentages of success, playing the percentages unless there is a surprise element. BB chats with coaches like Urban Meyer to keep current on prospects, etc. As Edison said: "1% inspiration - 99% perspiration."
BB was a DC, but he made an effort to learn offense and special teams once he was past that....I suspect coaches like Rex Ryan did not make as large an effort.

Benjamin Franklin said:For the want of a nail the shoe was lost,
For the want of a shoe the horse was lost,
For the want of a horse the rider was lost,
For the want of a rider the battle was lost,
For the want of a battle the kingdom was lost,
And all for the want of a horseshoe-nail
