Well, when Pioli left they essentially split the duties in two between Caserio and Floyd Reese. When Reese left after the 2012 season, Caserio basically became the "GM".
Lombardi's role is really intriguing. I get the sense that BB interfaces with Lombardi to work with football ops on various things. Personnel, scouting, etc.
As an important note, though, he is not officially a GM, because he does not have final say over the roster. This is important because, even with his contract, the Patriots cannot prevent him from interviewing for, or accepting, any position that would give him such control.
