These seem to be grayed-out with Firefox. Chrome is ok. Could be just my computer...not sure.

Obviously way late in answering this, but that's likely because the BB code icon was toggled:


Happens to me occasionally as well and you're right, not exactly self-explanatory so hopefully adding this response might help a few people if anyone runs into this.
