Is there an explanation, somewhere, as to how the NFL decides which penalties get called?

(A) Denver-Pats, Denver kicking: Three penalties, two against the Donkeys, one against the Pats--penalties offset, repeat kick

(B) Denver-Pats, Pats kicking: Two penalties, one each side--penalties offset, no repeat kick

(C) Haynesworth called for unsportsmanlike conduct, and then for removing his helmet--both penalties are enforced (creating the bizarre spectacle of Dallas kicking off from the TEN 40--something I'd like to see).

[I do realize that if one team gets called for multiple penalties on a play, that normally the aggrieved team gets to pick which one, if any, gets upheld.]
