What can this team do to avoid yet another 4th quarter collapse?

Hey, I don't have enough posts to start a thread in the regular forum, but have been here for years.

These road game collapses (Denver, Indy, Miami, Houston- forgetting any?) have been horrible to witness.

What can they do differently to put away teams and finish games?
More substitutions during the game to rest the defense?
Better conditioning to last four quarters?

Why is there such a drastic difference in how they play at home versus on the road?
