I'm baffled as to how players don't wear mouthguards mow that i think about it, i rarely see them doing so. Even qbs should wear them. I think it would cut down on cte as well.

I don't think they all do, but some definitely do. You can often see them attached to the facemask by a bendy bit of plastic. From personal experience, they can be uncomfortable to wear when you are breathing hard, and obviously hinder communication. The players have facemasks anyway which presumably offer a lot of protection to the teeth.
