I *think* you don't have to if they're lined up on the end and there's no WR on LOS... but I'm not sure

How can Costanzo be an eligible receiver when:

1) it wasn't announced
2) He lined up in the normal tackle position, not outside the normal 5

Absolutely. The Patriots Pass D is competitive when the coverage figures out the assignments. Right now a few blown assignments have really hurt. Hopefully this is something that get's better/doesn't happen as much each week....
