I don't think its fair to conclude that that is their thinking. If its fair to say that combat changes everyone then its fair to say that everyone gets PTSD to a greater or lesser extent IMO. That would mean that everyone automatically deserves a Purple Heart the moment they arrive on the battlefield; but if that's how the medal was awarded it wouldn't have any meaning.
Keep in mind too that there are no extra-Purple Hearts. The guy who suffers a minor flesh wound that's nothing but a small scar a month later gets the same Purple Heart as someone who loses a limb. That may not be particularly fair, but its clear and rational.
